refactor(mcp): change name to RawName and have method for Name() that is normalized

Raw Name is the name as it was parsed from the tool json
Name gives a name that uses kebab case and strips sg_ prefix ie. sg_commit_search becomes commit-search

Test plan

executed locally and unit tests use it
