feat: add multimodal runtime and operator tooling

• add native execution backends, background process management, and tool preset / approval routing
• add persisted automations, session search, user profiles, todo tooling, and review-first learning proposal flows
• add Gemini-backed vision and TTS tools plus a live websocket path with CLI/TUI/client support
