Fix size followup wrapper and approval gate

• Forward the under-target quality-increase flag through the app proposal-alignment wrapper used by Folder Studio preview.
• Let final profile approval accept under-target samples that actually raise quality targets without requiring explicit smaller-size tradeoff confirmation.
