Add client_hostname to Default and Azure session templates so events include the client host. Expose hostname in shared view template and the VS Code extension (model, panel, and view) and add a Host column. Also include small WinForms fixes: show InitialCatalog in recent connections, format AuthenticationMode display, and bump WinForms app version.
