Implement full extension scaffold: TypeScript source, JSON-RPC client and webview UI, package.json and build/test scripts, ESLint and tsconfig, unit test harness, docs (README/IMPLEMENTATION/SETUP/TROUBLESHOOTING), icon and packaging ignore files, and gitignore for the extension.
