docs: add complete structured coverage for all skills

Summary

• update README skill catalog to include all current skills with clear categories
• add planner/executor compatibility table with dependency semantics
• refresh CLAUDE.md taxonomy and orchestration patterns to match current repo contents

Why

Current docs had partial/outdated coverage (missing several active skills and execution modes), which made planner/executor selection ambiguous.

Scope

Docs only (README.md, CLAUDE.md). No behavior changes.
