feat: v3 architecture cleanup

Summary

• decomposes route internals and moves subscription lifecycle decisions out of route handlers
• adds v3 initialize result discriminants plus metadata helpers and docs/example updates
• hardens CI/package verification and TanStack Cloudflare example gates
