Adds src/hotswap/HOTSWAP_CONVENTIONS.md capturing hotswap-only rules: patch-pass authoring invariants and LIT test requirements. Split out from #2448 so the AGENT_CONVENTIONS.md updates can land without waiting on the hotswap source reorg. The doc lives at amd/comgr/src/hotswap/ to sit alongside the hotswap source files (a separate PR is creating src/hotswap/ for the source reorg).

Per Chinmay's other comments, this can wait on the hotswap source reorg before merging — the rules in here may shift with the refactor, and the content is also a candidate for cleanup toward more generic guidance.
