Improved the existing PULL_REQUEST_TEMPLATE.md by adding accessibility checklist items, better contributor guidance, and improved PR review structure.

Thanks @Mohitswamii! The structure improvements (accessibility checklist, how-to-test guidance) are good additions to the template. However, there are a few issues:

  1. The ## Summary section has been filled with content specific to this PR instead of remaining a generic placeholder. The template should say something like Brief description of what this PR does.
  2. Closes #1107 is hardcoded — the template should keep Closes #<!-- issue number -->
  3. The checklist items are pre-marked [x] — they should stay unchecked [ ] so contributors fill them out

Please update the template to keep these as generic, unfilled placeholders while keeping the structural improvements (accessibility section, additional notes, etc.).
