Unify git action flows and reduce sticky thread errors

Summary

• unify quick action and menu-triggered git flows behind the same action-specific dialog
• keep full git functionality while making push and PR flows explicit and consistent
• only surface thread errors when the session is actively errored, instead of keeping historical failures sticky

• local git diff --check passed
• bun fmt, bun lint, and bun typecheck could not be completed in this workspace because oxfmt, oxlint, and tsc were unavailable locally
