Batch CD notifications to the same ref #141
Description
If you configure a repo to CD to multiple environments, on the same ref, you'll get a separate DM from SlashDeploy for each environment. This gets kinda annoying if you deploy the same ref to a lot of environments.
It may be better if these could be grouped/batched by environment + required contexts.
