Document known issues and their resolutions

As #176 shows, we could have done a better job at documenting the bug and its resolution, that we encountered years ago in #97 

More generally, our multiple experiences implementing ReadiumCSS in apps and toolkits on different platforms could help others save a lot of time. 

A dedicated section in docs could be useful to surface common issues and options to resolve them.
