[LSP]: add symbol-provider / outline

this is likely similar to #8, the goal would be to be able to use "go to definition" (a `define`'d function, a `set $var`) by [`registerDocumentSymbolProvider`](https://code.visualstudio.com/api/references/vscode-api#languages.registerDocumentSymbolProvider) and to show those in the outline as well
