Refactor Image picker modal to use new Media Manager

To bring the plugin up to date with 3.5's new Media Manager, we should refactor to use the shiny new Media Manager for picking images.

FWIW, I looked at doing this, but it might be a be more than I want to bite off. The new media uploader looks super easy by itself, just not sure how to weave it into the existing code (image sizing stuff).

@benhuson Huge thank you for this! I looked at doing this long ago and it was over my head. It's on a list I keep of "Things I'd like to contribute to WP", but it's a ways down the list and now I can take it off and focus on the other things, yay!
