Libraries can be filtered by parental rating, but there is no way to pull it from the available metadata or set it manually, neither "Parental rating" or "Custom rating" fields are active.

You're correct, no idea how I missed that. And it appears it's accepting Jellyfin-shaped response, is that correct?

One other thing that could be addressed though is that "Custom rating" is not yet implemented, but I see a very easy path to get it working.

1. Migration to add column custom_rating to media
2. Update Media with field custom_rating
3. Populate when building BaseItemDto
4. Include it in PatchItemRequest

Then, in the smart library filters:

• override max_parental_rating with custom_rating if available
• for a list of certification ratings COALESCE(custom_rating, certification) so that custom_rating would take priority if available

Custom rating values to display in the Custom rating are available in the localization files.

tmdb very often does not have parental ratings data. ironically for the content that require it like adult themes content in languages other than english

Jellyfin workround is to block items without parental ratings. (its not foolproof or ideal)

Might consider implementing other custom sources for parental ratings or replicate the jellyfin approach for filtering
