Promote changelog to platform scope and add Unfold entries

Move changelogs.md from code-reviews/ to top-level so the page covers the kluster.ai platform broadly rather than only Code Reviews. Add three Unfold entries (Chrome extension, light mode, closed beta) with inline links to external destinations. Lightly disambiguate 17 existing entries that became ambiguous after the scope broadening, and update frontmatter plus intro paragraph to reflect the new platform scope.
