Audit and update stale documentation

Summary

Several docs in the repo may be stale after recent changes (v0.4.17–v0.4.23), especially the lodge-first onboarding PR (#303).

Files to audit

• HUMANS.md — user-facing docs, likely references old CLI onboarding flow
• ARCHITECTURE.md — may not reflect current state model, session registry, or onboard mode
• CHANGELOG.md — may be behind recent releases
• ROADMAP.md — priorities may have shifted
• DEV_FLOW.md — dev workflow docs
• SESSION-NOTES.md — may have stale context
• CLAUDE.md — platform instructions for wolts
• template/CLAUDE.md — template for new wolts
• docs/migrations/ — check if v0.5.0 migration is accurate
• lore.md — creature types may need dog/wolf updates
• container/bot/AGENT-LOOP.md — bot architecture docs

What to check

• References to old CLI wolt naming flow (removed in Lodge-first onboarding — auth and wolt creation happen in the browser #303)
• References to throwaway auth container (removed in Lodge-first onboarding — auth and wolt creation happen in the browser #303)
• Stale architecture descriptions (state model, session registry, sites vs apps)
• Missing documentation for recent features (telegram reply routing, tmux paste-buffer, onboard mode)
