[ARI-37] Acceptance test report and iteration log

As a Data Platform OPS engineer, I want a documented acceptance test report capturing pass/fail results for each criterion so that we have a clear record of what passed, what failed, and what was fixed.

Acceptance Criteria

Given all 10 test incidents have been processed
When the report is written
Then it includes a results table showing pass/fail per incident per AC (AC-01 through AC-06)

Given any AC fails on the first run
When the failure is investigated and fixed
Then the fix is documented with: root cause, change made, and retest result

Given the final report is complete
Then overall pass rates per AC are stated and the Phase 1 definition of done (all 6 ACs passing on 10 consecutive incidents) is confirmed or not
