Search results dialog styling

restyle the search results:

• Blue header
• Each record has;
  • grey subheader containing hospital number
  • service user name on right,
  • service user date of birth on left
  • X to close window in header

Changed layout of search results popup in create appointment user search and on main Service User Search View.
