Logging notification data

The goal is to log the user response (yes or no) in the database.
Acceptance criteria:

• Mash will show Sarah and Eura where the file is for logging these notification and relevant steps
• Eura and Sarah will set up the MySQL structure to receive this data
• The MySQL data is accepting and storing the notification responses.
