allow `start_at` argumente for /podcasts/dead enpoint to leave through results

I appears as if there was no way to retrieve all dead podcasts using the /podcasts/dead endpoint. Maximum number of results is 10000 and it's not possible to get more results. Would it be feasible to allow the start_at argument?
